NON-RESIDENT UTAH CONCEALED FIREARM PERMIT CLASS LIABILITY WAIVER

I acknowledge that this is a non-resident Utah Concealed Firearm Permit class. I acknowledge that I am a student attending the non-resident Utah Concealed Firearm Permit class.

I acknowledge that I have received basic and not complete instruction concerning the State of Utah’s laws regarding concealed firearms for non-residents and the use of deadly force, and interpretation of the law rests upon the student. The same applies to any other resident or non-resident permit to carry class.

I acknowledge that this is not an NRA© class, and has nothing to do with the National Rifle Association©.

I acknowledge that Lock and Load Academy, LLC, United States Shooting, the company, the corporation, this non-resident Utah Concealed Firearm Permit class, and instructor Matthew F. Wilson do not accept or assume any liability or responsibility what-so-ever for any  acts or omissions that are purportedly the result of this class. This shall be applicable to all students, visitors and the venue.

I acknowledge that the non-resident Utah Concealed Firearm Permit is recognized in many states. Each state has their own laws and firearms regulations. The individual state laws of other states are not taught in this class. It is the responsibility of each student to know the applicable laws, firearms safety rules, and to practice safe weapons handling at all times. Be aware that this class briefly discusses laws and is in no way a complete explanation of any state or Federal law. It is the responsibility of the student to understand and comply with each and every state and Federal law where one carries a firearm.

I acknowledge that it has been explained to me that Utah’s non-resident Concealed Firearm Permit reciprocity agreements between states’ attorneys  general offices change, and it is my responsibility when I am traveling, to contact the attorney generals’ offices of the states in which I intend to utilize the non-resident Utah Concealed Firearm Permit, and for the most current information on the recognition of my non-resident Utah permit. I attest that I am not a resident of the State of Utah.

I acknowledge that it has been recommended to all students after completing this class that they seek additional firearms training, to include live fire instruction by a competent instructor and to seek legal advice before carrying a concealed firearm in any state or jurisdiction.

This class has a no refund policy.  Our class and accommodations are arranged on a student to student basis.

I recognize that final approval rests upon the Utah Bureau of Criminal Investigation (BCI). Completion of this class does not guarantee a non-resident Utah Concealed Firearm Permit. All pre-requisites must be met, per the Utah BCI. Permits are granted based on properly completed, submitted, and approved applications to the BCI of Utah. The same holds true for any other states’ concealed carry permit application.

I understand that my non-resident Utah Concealed Firearm Permit application is not copied or stored by the instructors or the company, and that only my name, address, email address and phone number may be kept by the instructors. Vital personal information such as social security numbers, driver’s license numbers and credit card information are not kept on file.

I understand that this class covers excerpts from the State of Utah and the United States of America’s federal laws relating to concealed firearms and transportation, and is not legal advice. The instructors have not given any legal advice either verbally or through class materials. The company and instructors are not suggesting that I carry a concealed firearm, use a firearm in self defense, or own a firearm for any reason.

I understand that this class has nothing to do with purchasing, using, or owning a firearm in the State of New Jersey or Pennsylvania specifically, and states other than Utah.

I acknowledge that any non-resident Utah BCI certified instructor has the right to dismiss me from the class and/or eject me from a class for any reason, and that my class fees will be refunded in full within 15 business days.

I consent to having my fingerprints recorded by ink on FBI Applicant form FD-258 with ORI designation “UTBCI0000 BU OF CRIM IDENT SALT LAKE CITY, UT” and that the form was not duplicated or copied by any means by the company or instructors. I am the only person who has the original fingerprint card. I understand that I am to submit the fingerprint form/card to the Utah BCI with my application. If the fingerprint card is rejected by the Federal Bureau of Investigation or any other state agency, for any reason, the responsibility for reprinting will be upon the student. A clean blank FD-258 will be sent to the student by postal mail.  It will be the responsibility of the student to obtain satisfactory fingerprints. Rejected “passport photos” will not be replaced by the Company. It will be the student’s responsibility to have a passport photo replaced by using the services of another company other than Lock and Load Academy, LLC.

Stun guns and tasers or any other electronic device that delivers a shock to incapacitate a human being may or may not be regulated by state, municipal or any other law prohibiting such devices, including its use and personal possession. I affirm that I am responsible for any laws regarding the purchase of same from Lock and Load Academy, LLC and their usage.

Lost or misplaced student applications, stamped by the instructor, for the BCI, will not be reissued.

If a student application is “lost in the mail”, a new packet will be reissued only if the student can provide a delivery confirmation and/or tracking number proving it was not delivered, but mailed. Arrangements can be made to completely re-do the application and fingerprinting on mutually agreeable meeting terms. A photograph will not be reissued under any circumstances.

I understand that I have a year to submit my application to the state of Utah. If a year lapses it will be the responsibility of the student to attend another Utah class for recertification, at the current class cost.

I have received a copy of the “Minimum Training Curriculum for Utah Concealed Firearm Permit Class” as published by the UTAH BCI in either hard copy or digital form (DVD/CD).

I will send in the Utah non-resident application signed and dated by the instructor, within 1 year. If I fail to submit the Utah non-resident application to Utah within one year, it will be void and I will have to take another class and reapply.

Arrangements can be made to completely re-do the application and fingerprinting on mutually agreeable meeting terms. A photograph will not be reissued under any circumstances. The student may call, write or email the Utah BCI to confirm that the application was not received and is not being processed. Utah is a closed record state and Lock and Load Academy, LLC can not speak to the Utah BCI regarding your application status or any other issue.

I will send in the Utah non-resident application signed and dated by the instructor, within one year. If I fail to submit the Utah non-resident application to Utah within one year, it will be void and I will have to take another class and reapply. If a year lapses it will be the responsibility of the student to attend another Utah class for recertification, at the current class cost.
